The community-based Integrated Care System Model. By 2025, when the baby boomers will reach the age of 75 years old and older, a structure called “the Community-based Integrated Care System” will be established that comprehensively ensures the provision of health care including emergency care, nursing care, prevention, housing, and livelihood support. The elderly will be able to live the rest of their lives in familiar environments and be accommodated for their long-term care needs [27, 28]
